I was very fortunate to apply and be accepted to a fellows symposium under the sponsorship of my PI. Would this be something that I could include in my W/A section? If so, would it make sense to put it under "conference attended" and what would adcoms want to see from an experience like this? Thank you!

What will your role be? Are you presenting?
 
Conferences attended is a category in AMCAS but it is generally not considered a big deal. If you are presenting, then the better category is presentation. Mostly, in my opinion, it looks like filler.

The symposium is for fellows and faculty to gain insight from specialists. It is not the typical conference where there are research posters, but rather, a series of talks and seminars. I did have to get a recommendation and submit a case study as part of my application.

This sounds on-par with taking university seminar classes, which I doubt you will list in the Work/Activities section. This is not what Conferences Attended would be used for. If you learn something impactful from the conference that will affect your research, you might mention it in your Research space, but I doubt it will get you any traction with adcomms.

What will happen to this case study at the symposium? Are you presenting it? Is your PI presenting it? Something else?

I'm choosing a Canadian example because chances are you aren't going to Quebec. Is it something like this program?

My suggestion: let your PI bring this up in their LOR for you. You don't need to use a space in W/A unless you are presenting.
